　　PROCUREMENT OF MEDICAL EQUIPMENT

　　1. The Federal Democratic Republic of Ethiopia (FDRE) hasreceiveda grant from the Italian Government towards the cost ofHealthSector Development Program and it intends to apply part oftheproceeds of this grant to payments under the contract fortheprocurement of Medical Equipment as defined in the WorldBankGuidelines and according to the Italian Ministry of ForeignAffairsrequirements.

　　2. The Ministry of Health (MOH) now invites sealed bidsfromeligible bidders for the supply of:-Lot No. DetionIMedicalEquipment for Assela HospitalII Medical Equipment forOromiaRegionIII Medical Equipment for Afar RegionIV MedicalEquipment forSomali Region

　　3. The bid will be conducted through InternationalCompetitiveBidding procedures specified in the World BanksGuidelines andaccording to the Italian Ministry of Foreign Affairsrequirements.The Bid is open to all eligible bidders from eligiblesourcecountries as defined in the World Bank guidelines.

　　4. A Complete set of bidding documents in English Language maybepurchased by interested bidders on the submission of awrittenapplication to the address below and upon payment ofanon-refundable fee of Birr 150 (ETB) if locally purchased, orEURO30.00 or equivalent in any freely convertible currency ifpurchasedfrom abroad. This cost (EURO) includes the expenses forsending biddocument/s by courier service to overseas bidders.Additionalcopies can be purchased at same non-refundable feementioned above.Delivery of the tender document will be effectedafter receivingthe purchase request and remittance of the saidamount tot eh MOHA/C No. 0160101011900 at National Bank of Ethiopia45 days prior tothe bid submission date.

　　5. Local Agents and local manufacturers shall providerenewedtrade license and Registration Certificate from MOFED andshallindicate their VAT numbers.

　　6. Interested eligible bidders may obtain further informationandinspect the bidding documents at the Office of Secretary oftheTender Committee, Ministry of Health, PASS, Room No. 1.3, Woreda3,Kebele 53, Addis Ababa, Ethiopia during working hours from8:30A.M. to 12:30 P.M. and from 1:30 P.M. to 5:30 P.M. from MondaytoFriday.

Bid must be delivered to the address below on or beforeMarch31, 2005 at 2:00 P.M. local time and must be accompanied by abidsecurity of not less than 2% of the total bid amount in a formofBank guarantee, or Certified cheque.
